Transplante pulmonar duplo: considerações técnicas e aplicação clínica

RESUMO

A reintrodução do transplante pulmonar unilateral como modalidade clínica e a experiência com o transplante cardiopulmonar propiciaram o desenvolvimento do transplante pulmonar duplo como tratamento da doença pulmonar no seu estádio terminal. Embora este procedimento se encontre em fase inicial, os resultados preliminares são satisfatórios. O risco de deiscência da anastomose traqueal pode ser significativamente reduzido com a proteção dessa área por epíploo. As vantagens do transplante pulmonar duplo incluem remoção de todo o parênquima pulmonar doente, enquanto o coração é mantido inervado. Adicionalmente, é evitado o risco de aterosclerose acelerada. Os progressos na área da preservação pulmonar e um diagnóstico preciso de rejeição deverão influenciar nos resultados, a longo prazo. O transplante pulmonar duplo representa um grande potencial no manejo da doença pulmonar terminal e esta técnica deverá auxiliar na utilização racional dos poucos órgãos hoje disponíveis para transplante.

ABSTRACT

The re-introduction of single lung transplantation as a clinical reality and the experience of heart-lung transplantation prompted the development of double lung transplantation for end stage pulmonary disease. Although this procedure is still in its infancy, preliminary results are encouraging. The hazard of tracheal anastomosis dehiscence can be significantly reduced with an omental wrap. The advantage of double lung transplantation includes the extirpation of all diseased pulmonary tissue whilst retaining an inervated heart. In addition, the long term risk of accelerated cardiac atherosclerosis is avoided. Progress in the area of lung preservation and more precise diagnosis of rejection should influence the long term results. Double lung transplantation offers great potential in the management of lung disease and may help to optimise the availability of scarce donor organs.
